Advancing Black Wellness Through Equity, Respect, and Action

At St. John’s Community Health, we are deeply committed to improving the health and well-being of Black communities across Los Angeles, Riverside, and San Bernardino Counties. We recognize the systemic barriers and disparities that continue to impact Black lives—and we are working every day to change that reality.

Our Commitment Is More Than a Promise—It’s a Practice

Health equity is at the heart of our mission. Through culturally responsive care, targeted programs, and trusted community partnerships, we are taking action to close gaps in care and build a healthier future for Black individuals and families.

What Our Program Entails

  • CBWH Mission
    CBWH provides comprehensive support to Black women and their families, particularly during the prenatal and postnatal phases. Our program offers a wide range of services, including prenatal and postnatal healthcare, access to dieticians, dental and vision care, mental health services, Pediatrics, personalized case management to assist with goal setting and connection to community resources. We also provide free weekly baby supplies.

  • Who Qualifies for CBWH
    One of the key features of CBWH is its holistic approach, serving as a one-stop shop for participants. Expecting or current mothers with a child under one,  and registers as patients with St. John's are eligible to enroll in our program. Participants receive prenatal and postnatal care, mental health services, pediatrics, dental and vision care, at any of our clinics.

  • How CBWH Works
    Our program consists of weekly in-person and or Virtual support group sessions, spanning 16 weeks, which include 8 weeks of prenatal courses and 8 weeks of postnatal courses, lead in a support group setting with meals fully catered for mommies and their nurturers and or children. After every session attended mothers walk away with a case of diapers, and wipes . Additionally, we provide case management services, and  transportation assistance is available for those in need who wish to attend in-person. After 8 sessions mommies receive a car seat and after 16 sessions, they receive a playpen. While participating in our program, mothers are gifted with donations from our community partners and participate in our baby showers.
