What We Can Help With

We connect you to the resources that support your overall well-being—whether it’s housing, food, benefits, or care coordination.

  • Assess & determine patient needs & goals

  • Provide assistance and/or referrals to address homelessness

  • Develop care/service plan as needed & support progress toward goals

  • Provide linkages to medical, psychiatric, social, food, educational and other services as needed

  • Ability to literally meet the patient where they are in order to conduct case-management services

  • Work with medical providers and patients to implement treatment and management plans by addressing the barriers that prevent patient from meeting health goals

  • Assisting in establishing benefits or income support
