RISE Reentry Program — Rebuilding Lives After Incarceration

At St. John’s Community Health, we believe everyone deserves a second chance—and the support to make it count. The RISE Reentry Program helps individuals returning home from incarceration rebuild their lives with dignity, connection, and care.

Through personalized case management, peer navigation, and community-based support, RISE empowers people to overcome barriers, regain stability, and thrive in their health, relationships, and futures.

What We Offer

Our reentry services focus on whole-person care, meeting each individual where they are and walking with them on the path forward.

  • Peer Support & Navigation
    Guidance from team members with lived experience who provide mentorship, understanding, and trusted connection to services.

  • Case Management
    One-on-one support to create personalized goals and connect with health care, housing, employment, and other resources.

  • Behavioral Health Services
    Mental health counseling and substance use support in a safe, judgment-free space.

  • Primary & Preventive Health Care
    Access to medical services through St. John’s clinics, including checkups, labs, and chronic condition management.

  • Employment & Education Referrals
    Help connecting with job training programs, legal support, and opportunities for personal growth.

Community Advocacy & Civic Engagement

We believe those most impacted by incarceration should be leaders in the fight for change. Our advocacy strategies include:

  • Phone banking and letter campaigns to elected officials

  • Community petitions and awareness events

  • Town halls and public testimony

  • Direct actions and visits to decision-makers’ offices

  • Story collection and public education about the importance of reentry services
